Schedule: Part-time, ~24 hours per week (Set schedule covering weeknights 5pm-8am, weekends in 12-hour shifts)
State License Needed: Multi-state licensure required (Minimum of 2 active licenses)
Medix is seeking a remote Nurse Practitioner to join a national after-hours care team supporting residents in senior living and skilled nursing settings. This is a fully remote position responsible for delivering telephonic and virtual care during evenings, nights, weekends, and holidays to manage acute concerns and safely reduce unnecessary hospital transfers. Essential Responsibilities:
• Providing after-hours clinical support to patients and facility staff via phone and telehealth platforms.
• Assessing and managing acute, chronic, and behavioral health concerns (acute changes in condition, falls, medication needs).
• Coordinating care across settings, including communication with facilities, hospitals, and outpatient providers.
• Conducting thorough clinical assessments and documenting all encounters in the electronic medical record.
