Physician / Family Practice / California / Locum or Permanent / Family Medicine (FM) Locums Job

• Work Dates Needed: ASAP - on going at least 3 months 
• Preferred Schedule: Monday - Friday 8am to 5pm 
• Worksite Setting: 100% outpatient - could go to multiple locations: 
• PACE CENTERS
• Scope of Work: 3 to 5 patients a day as the MD will collaborate with other professions for the care of 55+ and older.   This is primary care position & Training is provided on all aspects of the unique PACE model of care which provides all-inclusive coordinated care.  Center includes all medical, dental, rehabilitation, home car, eye care, senior gym on site.   Provider will work with other providers & therapist for the care of senior health.  This is an Elderly Day Health Center.  
• Licenses, Certifications, Requirements: CA license, DEA, BLS, Board Certified IM or FP - might consider BE if physician has clean record.   
• EMR: E-Clinical Works.   Initial Training & on board of all providers 
• Pay Rate Preferred: $150/hour plus we bill expenses
