QUALIFICATIONS/REQUIREMENTS:

• B.S. Pharmacy, Pharm.D. preferred
• License or eligible for license as a Registered Pharmacist in Connecticut
• Inpatient hospital pharmacy work experience preferred
• Post-graduate pharmacy residency or fellowship training preferred

Major Accountabilities/Critical Responsibilities:

1. Prepare and dispense medication accurately following protocols.
2. Provide timely drug information to healthcare professionals and patients.
3. Conduct medication counseling sessions for patients.
4. Deliver quality pharmaceutical care to all patients.
5. Ensure therapy appropriateness for all patient demographics.
6. Monitor patient therapy according to departmental policies.
7. Identify and address medication-related issues promptly.
8. Manage drug distribution and controlled substances.
9. Supervise pharmacy support personnel.
10. Complete required training and education.
11. Ensure cost-effective medication therapies.
12. Assist with pharmacokinetic consult services.
13. Follow therapeutic substitution policies.
14. Participate in quality improvement activities.
15. Contribute to hospital initiatives and committees.
16. Adhere to 340B program requirements.
17. Demonstrate fiscal responsibility.
18. Follow regulatory standards.
19. Stay updated on drug use through continuing education.
20. Precept pharmacy students when assigned.
21. Provide drug information in-services as needed.
22. Contribute to quality initiatives as assigned.
23. Work flexible shifts as needed.
24. Communicate effectively with healthcare professionals.
