Center for Comprehensive Health Practice

<ul> <li>FT/PT</li><li>Hours: Monday through Friday | 8 hour shifts, 5 days a week | Part-time is 3, 8 hour shifts a week.</li> <li>Employed</li> <li>New Graduates</li> <li>Average Patients seen: Varies</li> <li>Call Schedule: Weekend and Evening call by phone as assigned.</li> <li>Loan Repayment</li> <li>Compensation: &#183; $210,000.00 - $230,000.00 annually</li> <li>Benefits: &#183; Health/ Dental/ Vision insurance &#183; PTO- First year - 15 days vacation. Years 2 through 9 - 20 days vacation &#183; 13 days sick a year and can accrue a max of 30 &#183; Flexible spending account &#183; Life and ADD insurance: 100% sponsored by company- max at 50K &#183; Retirement plan &#183; Malpractice and any board certification fees covered &#183; Loan Forgiveness offered &#183; LTD- 100% sponsored by company (To be eligible, Full-Time Employee must work a minimum of 21 hours weekly)</li> <li>Additional Info: RESPONSIBILITIES: &#183; Provide medical care in line with medical training to patients at the Center. &#183; Responsible for comprehensive recording of work-up activities, diagnostic findings, medical plans, & treatment follow up. &#183; Order, interpret & evaluate diagnostic tests as needed. &#183; Participate in multidisciplinary case reviews and family treatment in conjunction with other staff to formulate comprehensive treatment & rehabilitative plans. &#183; Participate in the development/ implementation of new services, trainings and quality improvement projects. - Mentor medical students at CCHP during their rotations, if applicable. REQUIRED: &#183; Computer literacy and EHR experience &#183; Board Certified or eligible, in Family Medicine &#183; Valid NY state Medical License, DEA, & BLS certification &#183; Ability to work effectively with diverse populations &#183; Commitment to CCHP's mission PREFERRED: &#183; 1 year working in a community health clinic or practice &#183; Addiction Medicine experience &#183; Spanish speaking </li> </ul>
