Aza Health - St. Augustine
<ul> <li>FT/PT</li><li>Hours: Monday - Friday 8:00AM - 5:00PM and no weekends!</li> <li>Employed</li> <li>New Graduates</li> <li>Average Patients seen: 18-20ppd for 8 hour shifts</li> <li>Call Schedule: Phone call only, about once a month</li> <li>Loan Repayment</li> <li>Sign-On Bonus: $12000</li> <li>Compensation: Annual salary is negotiable depending on experience with a potential range of $200,000-$240,000. This is dependent on hours worked and productivity.

JOB DESCRIPTION:
-Provide quality care with the number of patients seen per day of 18-20, offering telehealth to our distanced community, mitigating high no-now show rates or reschedules
-You will play a critical role within our community, providing not only primary care services but also preventive care.
-With our given population, you will witness growth within our community and limit complex cases from advancing, keeping our community healthy and limiting their need for emergency or hospital-based resources
QUALIFICATIONS:
-All FM providers must be able to see patients within the full lifespan (birth to adult)
-BE/BC
-State licensed
-Bilingual required for the St. Augustine site due to a high population of Spanish speaking patients
