    • Aug 21 @ 7:00 am - Aug 30 @ 7:00 am
• Clinical Information:
  • Emergency Department and Hospital consults and emergencies
  • Common Diagnoses/Procedures:
    Hydronephrosis with renal and ureteral calculous obstruction, Calculus of kidney,Benign prostatic , hyperplasia with lower urinary tract symptoms, Calculus of ureter,Urinary tract infection
  • Patient volume: 0-10 daily
  • Only 1 full time Urologist in FPN
  • Need additional coverage for Hospital not for the office
  • Only ED and Hospital work, no office
  • Cover emergencies and any needing surgery
  Required:
  • Must Use EPIC
  • Active IN license, DEA and CSR
  • Clean file to start in June
  • CV must be in Month / Year format
  • Must have license in hand at time of submission, or proof that the license application has been submitted
  • Any gaps greater than 30 days will require an explanation on the CV
  • DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
  • Current Flu Vaccine
  • Must be fully vaccinated for Covid 19
    • Exemptions only allowed for religious or medical reasons, once approved by the facility.
