Physician / Gastroenterology / Indiana / Permanent / Locums Gastroenterology Need in Indiana Job

• Coverage Needed:
  • May 1 2023 - Ongoing
  • 2 days/wk Amb. Clinic (Total 6-8 hrs./wk)
  • 2-3 days/ Endoscopy
  • IP consults daily

• Clinical Information:
  • Patient volume: 10-18
  • Support Staff: MA, LPN

• Required:
  • Active IN license
  • Board Certified
  • General GI procedures
  • CV must be in Month / Year format
  • Must have license in hand at time of submission, or proof that the license application has been submitted

  • Any gaps greater than 30 days will require an explanation on the CV

  • DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active

  • Current Flu Vaccine

  • Must be fully vaccinated for Covid 19
  • Exemptions only allowed for religious or medical reasons, once approved by the facility.
