We are looking for a Chronic Care Physician to join a premier home-based health system in the Philadelphia, PA area:
- Permanent, full-time position - Internal or Family Medicine Physician with Geriatric background preferred - Join a team that places an emphasis on quality of care with excellent support staff - Average of 5-6 patients per day - Excellent compensation and benefits plan - Minimum (2) years experience required - Very minimal call (Telehealth)
