• Requirements  
  
  
  
  • Activity logs (If the practitioner is requesting additional specialty procedures, they will always need activity logs to support this request)
  
  
  • Sedation Exam as they all get sedation according to Dr. Porter and the MSO (this is always sent by me)
  
  
  • Active MD license 
  
  
  • Active DEA/CSR
  
  
  • If APP is a recent grad- they will want a residency and fellowship director reference.
  
  
  • PALS if it's Pediatric Medicine
  
  
  • ACLS
