Description
We are recruiting a board-certified Family or Geriatric Medicine physician to join a multi-disciplinary practice in the greater Phoenix area.
* Employed, full-time position
* Outpatient opportunity
* Primarily elderly medically complex patient population
* Enjoy smaller panel sizes
* Actively participate in a collaborative Care Team with Health Coaches, Nurses, Nurse Practitioners and Behavioral Health Specialists
* Collaborative happy work environment geared towards patient-centered care
* Face-to-face office visits and non-visit based interactions (e.g. phone, email, groups, home visits, video chat)
* Competitive compensation with comprehensive benefit package
* Spanish language fluency preferred, but not required
* Community population nearing 140,000
* Practice is located 35 minutes from Phoenix center city
